The City Clerk's Office is the custodian of all city records.  For a records request form CLICK HERE.  Please print the form.
When you have completed the request, please submit it using one of the following options:

  • Fax to (318) 872-0402,
  • Deliver it in person to the Clerk's Office at City Hall, 705 Polk Street, or
  • Mail it to the City Clerk's Office, P.O. Box 773, Mansfield, La 71052.
